Each scope is checked with workspace, month and grantor role. Obtain expectedRevision from the latest pipeline_state or successful mutation. On STALE_REVISION reload and reassess; never blindly replay a write.

Quotas, processing capacity and disk admission can temporarily refuse work. Over MCP, AGENT_AUTH_REQUIRED is HTTP 401 and AGENT_SCOPE_REQUIRED is HTTP 403 with an insufficient_scope challenge (new consent needed); month, source and whole-workspace refusals and other tool failures arrive as a tool result with isError and the code first.
